The following is a list of acceptable proof of identification:

• Driver's License
• Birth Certificate
• US Government ID with DOB/SSN
• US Passport
• Certificate of Naturalization
• Certificate of US Citizenship
• Military ID
• Military Discharge
• Permanent Resident / Alien Card
• SSA-1099
• Statement of Benefits with DOB / SSN
• Tax Return or Tribal tax return
• Unemployment / Workers' Compensation Statement with DOB /SSN
• Weapons Permit with DOB / SSN
• W-2
